Parent(s) or legal guardian(s) who must provide transportation to and from school because free transportation is not available for their children may be eligible to receive money from the state to help offset some of the cost, for example, bus fares or mileage reimbursement for private automobiles at $0.505 per mile...

Please note (as explained in the NOTICE) if you apply for transportation reimbursement and live WITHIN 1 1/2 miles from the school, you need to file a Serious Safety Hazard form with the state. Please download and print this Application for Determination of a Serious Safety Hazard (which includes Forms and Instructions).

Or, you can contact the Illinois State Office for information at 217-782-5256. If you filled out the Application for Determination of Serious Safety Hazard within the last three years, you do NOT have to reapply.

These applications must be filed by February 1, 2008.
